Restorative Nurse (RN or LPN) New

The Restorative Nurse (RN or LPN) provides support to the nursing and therapy departments in assisting residents to achieve their highest level of function, maintain skills learned in therapy, minimize declines in function, and restore the level of functions lost through illness, debility, or lack of motivation. Conducts evaluation of residents for their need of a Restorative Program, writes a program plan and reviews the resident’s progress towards goals.

Responsibilities

• The Restorative Nurse (RN or LPN) provides nursing rehabilitation/restorative nursing services.
• Performs evaluations of residents need for individual Restorative Programs.
• Performs nursing assessments regarding the health status of residents. Notifies the physician of changes in status and promptly implements and documents new orders.
• Develops a plan of care based on assessment, implementing nursing/restorative care.
• Selects and institutes appropriate nursing interventions that might be required to stabilize a resident’s condition and /or prevent decline.
• Contributes to the resident’s assessment (MDS/CAA’s) and the development of a plan of care.

Qualifications

• RN or LPN license in the state
